We are happy to assist your family in applying for childcare vouchers or helping you sign up for our fee pay program.
To submit a voucher application, you will need:
- IDs for all adults in the home
- Social Security Verification for all individuals in the home
- Proof of residency
- Proof of birth for the child being enrolled
- Immunization record for child being enrolled
- Latest Physical / Well Child Screening for child being enrolled
- Valid email address

Voucher Applications can also be made online through the School Readiness Assistance Portal.
Once completed and all documentation gathered, feel free to drop off at Lake Point or Dardanelle locations.
Copies of applications are kept at both locations and can be completed on site, if needed.
